Those are called obfuscation mods. They exist, but they require extra computing power and can cause lag.

Intelli has anti-hack mods that he wrote himself. They work fine, and ban the flyers and the majority of xrayers.

I think it's safe to say 99.9% of the players do not have the required knowledge to bypass or reverse-engineer a server-side obfuscation mod. A player who did know how to do that could just as easily give himself god powers, or DDoS the server.
The funny thing about this is that in order to not get banned by help bot, a "hacker" (Sheesh I hate that word -.- ) would have to find/mine diamonds at about the same rate as a normal miner, which makes the hack useless because it is not increasing the total diamonds vs. the total time period.
As for it being a permanent ban: Last time I checked, the majority of the ban appeals are from randoms who have been on the server for 2 hours before being banned. And most of them don't get unbanned, or don't bother coming back to check on their appeal. Not the "repeat offenders" that people are talking about. If your talking about Kress, he's dead and gone, get over it.
If a player puts the effort in to show he is sorry, writes a good appeal, and is willing to stick around for a bit while the admins mull it over, I feel they should at least have a chance. If you look around, you'll figure out that a lot of players who are well known on the server have been banned before, generally for xraying. I'd throw names out there, but I don't want to bother stirring up things that happened previously.
TL;DR: Not everyone who is banned is a terrible person.
