If you make them permanent, then more and more alts/ phackers etc will be targeting mr.

MineRealm is the only server i ever go on. Only minecraft i ever go on. Actually, now that i look, ive only got one single player world.

Many players feel the same way. Maybe there could be like... A criminal record. The seriouser the crime, the more points u get, whenu get to a certain number of points u get perma banned.

I really don't like the permanent ban part... I'm too worried if I'd be banned, well, I would never get to play here again.

Change 1. I somewhat agree with this, although I think a yellow card/red card system could work, where if a player commits a minor offence for example destroying a couple of blocks of someone else's house they would get banned, if they appeal and their appeal is sincere they get a second chance, if the same player then commits another offence regardless of its severity they would get a red card and be perma-banned, on the other hand a more severe offence for example, where they destroy someone's entire house and loot all their items is a red card, meaning that no matter how sorry or how many appeals they make they don't get unbanned.

Change 2. 100% yes.

I voted yes as even without my suggestion on change 1, I still think this is a great idea and I personally will not be affected by this as I don't do anything that would warrant a ban.
