Okay I have had this happen to me once, and its also happended toTrigerhappy191. When teleporting either to or from Trade. If it is the case that the map doesn't seem to have loaded and you fall into the void before lossing health rapidily and dying.
This is a glitch. A glitch which I think is a problem with Minecraft itself. You've suffered a chunk load error I'm guessing. Possibly similar to what happens when if you've ever logged out in the nether and got stuck on the cavern roof upon logging back in.
When ever the map loads it will try to place you at your co-ordinates if there is space. In the case of the nether, there isn't always space and so it will place you at the next available place when the map loads, which may end up being above the map.
In the case of the /trade teleport. The I'm guessing the map data hasn't loaded correctly or in time and been disrupted, just like the old world holes. As a consequence you fall through the world into the void and die.
Solution:
1) Prevention measures
When teleporting either to or from trade always make sure your render distance is down to tiny. This reduces the amount the map has to render, thus reducing the likehood overall of a chunk load error. I also reccomend turning down all the other graphics options to improve performance. You don't have to play the game like this constantly but I reccomend it for teleporting to trade, especially when carrying valuables.
2) What to do if it occurs
This is theoretical more than practical. Quickly recognise that the glitch has occurred and LOG OFF. This should solve the immediate problem of dying. Then make sure all the actions in 1) are taken. Then remeber where you were. If in Trade, quickly type /trade, immediately upon logging in. By teleporting to a new location there is a better chance that the map will load. Be prepared to loss some health in anycase as you may land in the ground and suffucate first.
You may still die upon logging back in, which is why it might be good to type /home, assuming it has been set (advice to all players in general). That way if you die, atleast you should die on the surface and be able to collect your stuff.
