I tp'ed home, but upon arriving, I started to fall and take damage. This happened earlier and I died when I logged back in. Is there something that can be done to save me, because I have full diamond armor and tools on me?

When I start falling, I immediately log out and then log back in. I have never taken damage or died from the glitch. If you don't die immediately when you log in, you can try another TP like /trade.

Not sure what else you can do...

Reduce render distance down, right down to Tiny.
However if you're currently beneath the map, you'll need to immediately type /home or /spawn as soon as you log in.
Having the render distance down to tiny reduces the overall chance of a teleportation accident such as this occuring.
Also I recommend turning down all other graphics options down to lowest settings.
This will increase the likely hood of the map data transferring across to your client side without the world hole issue.
There is still a risk of dying however, but this method is the best I know of to date.

Falling into the void is glitch that I sure is caused by errors in the data being downloaded and uploaded from clientside to what ever server you play on. To my knowledge its a glitch to do with Minecraft itself rather than a minerealm one. Its to do with chunk errors.

Resetting you /sethome in a diifferent chunk may help reduce the chances of this occurring again.
But for prepared teleports from home to trade or when ever you want to carry large amounts of valauble items.

This won't help much at the moment, but I use something similar to the illustration below to keep chunk failures from killing me. There is a lot you can do to make the design more useful, but this is the simplest version.
