As far as I know the problem is not the villages combining after chunks unload, because the whole idea of the Iron Trench system is that it is capable of completely rebuilding itself if the system fails. That process is automated by the redstone contraption inside.
However, that is where the problems start to arise when trying to build this on MineRealm:
1. That minecart.
It has been said earlier on in this thread, the minecart in the system is a problem, as they despawn after 5 minutes. You could probably bypass this with either a piston powered elevator for the villager or a water elevator, similar to what people use in mob grinder. Downisde is that either will probably be slower than that minecart, which requires changing the timings in the system.
2. Village creation.
In an update video Tango Tek has mentioned that the total amount of villagers in the world heavily affect village creation mechanics. The Iron Trench relies on the fact that villagers can detect doors always within 6 seconds. However, if the total number of villagers in the world is over 100, it is more than 6 seconds. Possibly fixable by fiddling with the timings aswell.
3. Redstone on MineRealm:
It is well known fact that redstone contraptions do not work 100% flawlessly on MineRealm, probably on any heavily populated SMP server. I have tried to make an industrial smelter since quartz came out, but couldn't figure out what was causing it to fail. A friend told me to try and put the entire thing on a single chunk, thus limiting the amount of furnaces applicable. After some fiddling with reapplying hoppers the thing worked and has been working without any quirks or failures.
Basicly what was going wrong is the fact that a redstone signal does not always travel over a chunk edge, and even when it does, often it is delayed. As an Iron trench is much bigger than a 16x16 chunk (I think its closer to 2 realms, 50x100), there are going to be problems with the redstone wiring. It might work, it might not, its just going to be a lot of trial and error. My friends and I intend to try and build one, but I think it is not going to work.
4. Redstone loops
One of the moderators told me that redstone loops are frowned upon on MineRealm (maybe even forbidden) as they are one of the main sources of lag. The Iron Trench is basicly a giant redstone loop, loaded with repeaters, hoppers and pistons, so Intelli and the mods are probably not too happy about people building stuff like this. Sources of lag tend to get put to a hold on MineRealm.

Other than that, I think it is an interesting concept. If it is indeed possible to get it to work, it will certainly help to get rid of those ugly giant iron farms that mess up the scenery. A nice compact iron farm keeps space for other cool stuff like the 10 millionth skyscraper on MineRealm.
