I'll explain this for Red and for all wanting themselves in one of his animated scenes:

He takes your name, your reputation on the server, and your skin design to come up with a theme. He then attempts to find a high resolution background that could fit with the determined theme. If you aren't necessarily known for anything on the server then it makes it hard for him to determine the proper scene to put you in. On top of that, if your skin is a suit or any modern attire, it makes it near impossible to fit the minecraft character into something epic - office buildings are not epic.
