Honestly, I think the lack of enthusiasm with Minerealm has more to do with the slow development of Minecraft in general than anything with Minerealm specifically.
I don't see the Minerealm map as particularly small. Or laggy (you want laggy, play on a modded server for a while, uphill both ways in the snow!).
It's just that 1.8 took forever. It was compounded by the Craftbukkit drama. And even slowed a bit by the Microsoft acquisition. But there's also a very rich and vibrant modding community. Modders have taken the steps that Mojang should have been taking all along. They've made the leaps of creative development that Mojang used to put out.
* It shouldn't be so hard to organize your items. There's mods for that (Barrels, Better Chests, Advanced Energistics).
* There should be a better way to progress through innumerable aspects of the game, but specifically ore processing, materials creation, etc. There's mods for that (Factorization, Mekanism, Tinker's Construct, Thermal mods, Ender I/O).
* It shouldn't be so hard to farm and automate, and there should be tons more to farm. There's mods for that. (Growthcraft, Harvestcraft)
* There should be creative and beautiful brand new content. There's mods for that (Blood magic, Botania, Witchery, Biomes-o-Plenty, Roguelike Dungeons, heck - even Hats).
* There should be better ways of getting around and for moving items. There's mods for that (Enhanced Portals, Ender Storage)
* There should be better ways to really accent and accessorize your builds. There's mods for that (Chisel, Carpenter's Blocks, Natura)
I mean any problem you have with Minecraft being boring is being solved in mods. Mojang is simply too slow, and isn't innovating at a rate that is remotely fast enough for its easily-distracted demographic.
There is a happy medium between pure survival with its griefing problems, and pure creative with it quickly becoming boring. You can have a clear progression from nothing to quasi-invincible, nearly-creative access to materials - and make the entire middle part interesting, entertaining, and fun.
