Yes, it's possible. I almost bought a helmet that had both. However, it's fairly unlikely to get this result. In 1.3, the new enchantment balancing will probably make this a much easier task.

I tried it in creative like 50 times with gold helmets (best enchantability) levels 48-50, and didn't get it. So, if it even is currently possible, I don't find it worth trying to get legit.

I can't remember much anymore. It's been months since I owned it.

Well, I know that at one point during the 1.9 prereleases, all enchanted items got feather falling (including picks, swords, etc) although I think they lost all other enchantments. So meh. The game has to glitch out sometimes. And yes, I was referring to feather falling=boots, and Aqua Affinity and Respiration being helmet-y stuff, so...yeah.
