It is a common glitch, and there are no plans to fix it server-side. My best guess is that this is caused by a design flaw in the client/server interface. (the same flaw that makes speed/fly hacking and no-clip possible)
However, there are a few work-arounds you can try:
1) Place your /sethome on top of a 2-3 high pillar, then remove the pillar. This will often give the chunk time to load before you fall through. Extreme lag will still be problematic. This solution is 100% effective when using /home within 8 chunks of your /sethome.
2) Some people report that putting their /sethome in a place that doesn't allow them to see very far in any direction has fixed their issues. This doesn't seem to work for everyone.
3) If you do get stuck in a "world hole", try immediately relogging. It won't work every time, but it is very effective if you're following 1) and 2).
4) If you get stuck in a "world hole" and the server repeatedly kicks you for flying, you're only hope is to very quickly type /spawn or /trade. This will remove you from the world hole, and you may attempt to /home again. This issue is very rare, and I've never seen it twice in the same week.
The only real thing you have on the internet is your reputation.
