2 reasons why not:

1. Minerealm is a server with the original minecraft experience, many people don't want that changed.

2. For a mod like the moon mod (one that changes or adds blocks or mobs to the game) people would have to install the mod to their client. If they didn't their client would crash the instant they join the server. Since many people don't like molding their clients and even fewer know how, minerealm would loose a significant portion of it's audience.
