I was just running along and I "Fell out of the world" and died. I did lose a bunch of neat things, but thats beside the point. I just want to understand what happened? So this does not happen again. Is this a normal occurance? a player made trap? a glitch?

again any answer is fine, i would just like to understand for future reference.

They're commonly known as 'worldholes'. They are created when the chunk won't load. On some occasions you can die if you fall in one.

considering i play on quite a few servers and have yet to have this happen I'll be moving on.
