Me and redkiller123 where having a conversation about this helmet. He doesn't think it's possible and I do, I wanna know what everyone else thinks.

Aqua Affinity is for tools, so they can work better underwater.
Respiration is for helmets, so you can breathe longer underwater.

So nope, it is not, as one is for helmets, and one is for tools.

Yes, it's possible. I almost bought a helmet that had both. However, it's fairly unlikely to get this result. In 1.3, the new enchantment balancing will probably make this a much easier task.

My first thought was that is was like Silk and Fortune, not allowing same enchants, but after looking it up I found out that it's not impossible. You can in fact enchant a head-bucket with Resp and Auqa.
